How it Works
The Emergency Medical Form (EMF) app and website is a free service provided to the public and first responders.
For Individual and Community Use
The EMF app is available for download to anyone with a smartphone, it works just like our website.
Once you log in through the website or download the app, simply upload a picture and answer the questions provided on the form. Easily edit or upload any additional information to the form you would like to include.
Once your form is saved and up-to-date, simply open the app anytime you wish to access your form.
We encourage you to print out your form and use it as a backup for first responders who are not on or aware of the EMF system. You can also print a blank form here and fill it out using a pen or pencil. Place your printed form in a safe location and ready for use in case of emergency.
The EMF system is ideal for anyone who wants to protect themselves or their family during an emergency and we highly recommend it for senior citizens.
For Health Care Providers & Agencies
This version of the EMF app is available to download for agencies and health care providers. Ambulance services and fire departments can benefit from the use of this app to minimize the time spent on scene, improve the quality of care provided and in patient care reporting.
An agency or health care provider must first register and go through a verification process to use the EMF service. An agency can then either give access to their employees such as EMT's, paramedics, nurses, doctors and dispatchers or health care providers can individually submit the required credentials to download and use the app.
After gaining access, health care providers will have access to our extensive database of EMF. They can search and access the medical form of the patient they are responding to, either before or while on scene. This system also enables professionals to forward forms with their patient information across the network, such as between the dispatcher and first responder to the receiving hospital.
